Thank you for visiting our online registration. The Shifting Consciousness workshops offer life changing opportunities for participants. We are excited you have chosen to enroll in one of our upcoming workshops. To make the registration process as pleasant as possible the following instructions have been provided. Please take a moment to review this information before proceeding to the registration form.

If you have any difficulty submitting the online form please call 314.706.4882 and we will handle your registration by phone or e-mail us your contact information at registration@unifiedfieldtherapy.com and one of our representatives will call you within 72 hours.

Registration consists of three steps. All three steps must be complete to ensure class placement.

Step One: Complete & Submit Online Registration Form

Beginning the registration process is quite simple. Completethe registration form provided below and submit it to Shifting Consciousness, L.L.C.. When filling out the form be sure toprovide all requested information to ensure proper class placementand effective correspondence. Submission of the form is executed by clicking on the submission button located at the bottom of the page. Please double check your information to make sure it is correct before submitting your registration form.A confirmation notice will be sent via e-mail within 24hrs. to the e-mail address you provide, indicating your registration form was received.

Step Two: Providing Payment

Second step is providing payment for the workshop placement you have requested. In your registration form you should have indicated a method of payment. Your selection determines your next course of action. If your payment method is check or money order, you simply mail payment to the address specified below. Make all items payable to Shifting Consciousness, LLC. Note: If payment items do not contain the name provided on your registration form, please enclose this information in your mailing.

If you have chosen to pay by credit card the following online steps must be completed. First, after you have submitted your registration form you will need to click the back button on your browser. This will bring you back to this page. Scroll down to the bottom of the form and follow the link indicated which will take you to an instruction page with final details. Please read all instructions carefully before beginning the payment process. This will ensure your payment process flows smoothly.

Step Three: Placement Confirmation

Once the preceding steps have been completed, you will be contacted by a Shifting Consciousness representative to confirm your class placement. Contact will be made by phone or e-mail, one to two business days after payment is received.

Terms & Conditions

Cancellation Policy: If you must cancel, notify Shifting Consciousness L.L.C. directly at least 3 days prior to the class and you will receive a credit voucher, (less a $20.00 cancellation fee). Credit vouchers can be applied to any future S.C. seminar within a six-month time period from the date of issue.

In the unlikely event that a Shifting Consciousness, L.L.C. seminar is canceled, a full refund will be issued to registrant. Checks will be mailed to the address specified on participant's registration form. No refunds will be issued for courses that are postponed because of inclement weather or other circumstances beyond our control.
Cancellation of individual sessions with Instructor or Teaching Assistants require 72 hours notice for a full refund and must be phoned in to 314.706.4882.

Terms

Shifting Consciousness, L.L.C. makes no specific claims, promises or guarantees. Participants who attend classes should do so for their own learning and agree to the following conditions.

While participating in activities organized, sponsored or structured by Shifting Consciousness, L.L.C., I do hereby, for myself, my child, my heirs, executors and administrators, waive and release any and all rights and claims for damages I may have against Shifting Consciousness and its representatives, successors and assigns for any and all injuries suffered by myself at any activity sponsored by Shifting Consciousness, L.L.C..

I understand that the material presented in association with Shifting Consciousness workshops or other scheduled activities, is proprietary information. I agree to utilize said material within the terms allocated by Shifting Consciousness, L.L.C. and understand that presentation, duplication or modifications of this material is prohibited without the expressed written consent of Shifting Consciousness, L.L.C.
